Traumatic brain injury, also called TBI, occurs when the brain sustains a sudden trauma, usually blunt force. Because the brain is driven into the side of the skull by the force of the trauma, it suffers bruising, swelling, and potentially intracranial bleeding.

When the skull itself is damaged - broken or cracked - the injury is called "penetrating." This is less common than "closed" head injuries, which, while usually less serious, are more difficult to diagnose. "Shaken baby syndrome" is a classic example of the severe damage that a closed head injury can cause.
